
	TOP 50 RADIOLOGY GROUPS 2008 Application Thank you for your interest in the survey! We would like to compile the 50 largest radiology groups in the country based upon the number of full time equivalent radiologists. We will also publish the names of the lead executives, number of hospitals and imaging centers served and the number of procedures performed. We will be doing additional surveys in the future that will focus on imaging center companies only—be patient you will have an opportunity to be counted in the "Top 50 Imaging Center Companies" in the Spring of 2009. INSTRUCTIONS: Please complete this survey as your entry into the “Top 50 Radiology Groups” rankings. Submit data for the calendar year ending December 31, 2007. DUE DATE: September 15, 2008 SUBMISSION: The survey will be conducted online through LarsonAllen, an independent CPA, consulting, and advisory firm that will provide both system and data security. If you have difficulty with our online survey, please contact Jody Schuman at jschuman@larsonallen.com. The survey needs to be completed by September 15 to be included in the ranking.
